Requirement Change Request eForm

Form Details

This form is for currently admitted students who wish to select a new plan in their current program and/or who need a change to their requirement term. This form must be initiated by an Advisor; once submitted by an advisor, the form will be sent to the student for approval, then routed to the Admissions Office for processing.

Note: This link will only work for Advising staff who are logged into their staff PeopleSoft account. If a staff member outside of Advising or a student tries to access this form, they will receive an error message. Please refer students who need a Plan Change or Requirement Term change to their Advisor.
